Employees are provided Basic Life Insurance of $50,000 by the Board of Supervisors. Coverage includes Accidental Death & Dismemberment, Waiver of Premium for total disability, and the Accelerated Death benefit feature. Coverage reduces to $32,500 at ages 65-69 and to $25,000 at age 70 and over. Coverage terminates at retirement or when no longer eligible. This insurance is provided by EMC National Life Company of Des Moines, Iowa.
SUPPLEMENTAL EMPLOYEE TERM LIFE INSURANCE COVERAGE
Active employees insured under Basic Life Insurance plan can apply for additional Supplemental Life Insurance with Accidental Death & Dismemberment coverage. This will allow employees to purchase additional life insurance coverage by payroll deduction at favorable group rates and with simplified underwriting. Under this program, employees can elect any multiple of $10,000 up to $100,000. New employees can elect coverage up to $50,000 without any health questions if they apply within 31 days of becoming eligible. Requests above the guarantee issue limit and requests from late applicants will require Proof of Good Health.
DEPENDENT TERM LIFE INSURANCE COVERAGE
Active employees can elect Group Term Life Insurance coverage on their family members if applied for when first eligible. Eligible dependents are an insured’s spouse and unmarried children to age 19 or to age 25, provided such children are full time students and dependent upon the insured for main care and support. If any dependent is hospital confined on the effective date of insurance, coverage will become effective upon final release from the hospital.
